Furthermore, by submitting this potentially protected health information (PHI) to The Art of Online Prosperity, you agree that The Art of Online Prosperity may share such PHI with the specific health care provider from which you are seeking potential care, treatment, and education regarding a potential health concern.
In compliance with HIPAA, the HIPAA Privacy Regulations, and the Health Information Technology for Economic and Clinical Health Act of 2009 (HITECH), The Art of Online Prosperity:
